multiple monitor window placement

Hiya --

I have a problem that has been annoying me for far too long now, both in KDE (running 4.3 now) and gnome.
I have a multihead setup using nvidia twinview. My main screen is my LCD monitor, but I have a secondary screen running on a TV (s-video output) across the room from my computer for watching movies.
This works nicely for the most part, except for a pretty regular number of windows that appear on the TV screen and have to be dragged back onto my LCD (Firefox dialog window/popups, I'm looking at you!).

Is there any way to prevent any screens from ever opening up on the TV screen? (I can't even see the TV from my computer desk). Maybe using the 'window behaviour' settings in kwin?

I've seen this question asked periodically over the years, but have never really seen a good answer for it.
Disabling the TV output when not in use via xrandr is an option, but not my favourite one. Sometimes when re-enabling the tv ouput, my panels will get moved to that screen, which can be quite a pain to fix (this can also happen if/when plasma crashes, btw).
