sound system & notifications not working but soundcard does work

I run Slackware 11 with KDE 3.5.4. Soundcard is onboard ALi M5455 which is configured properly with ALSA.
If I view Flash movies on websites, sound is fine.
Watching vids through Kaffeine, sound is fine.
One day system notifications stopped working properly and I can't play music in things like Amarok or KsCD.
If I go to the Control Panel and choose 'test sound' I get nothing.
I can play OGG files like KDE_Startup_1.ogg from shell with ogg123 but no joy with anything GUI/KDE.
I've seen some stuff relating to .kde/share/config/knotifyrc but that hasn't helped - the settings seem "correct".

I created a test user and they can play music and things without any problems so I believe the problem to be somewhere in my ~/.kde config but there's so many files I can't see where the problem might be.

In playing around with system notifications, I've discovered it's not just the sounds that are gone. I set window maximise/minimise to play a sound and log to a file to see what was really happening. Absolutely nothing. Don't know if this is related or an unlucky coincidence.

I've seen plenty of similar problems on the forums but none quite match my symptoms. Any ideas would be much appreciated.
