kword custom paper size

I have a new OKI 6100DN color LED/Laser
printer that can print on paper up to 48"
in length. The "banner" paper from OKI
for this printer is 8.5" by 35.4" .

I have set a page layout to 8.5 x 35.4 and
created a document that I would like to
print.

I am using CUPS with the proper OKI 6100
PPD file.

I can not get the 35" banner to print. I get
a page of print approximately A4 sized.

If I print to a postscript file, I can not select
a custom page size, and the output generates
what looks to be an A4 sized output with
about 75% of the banner missing.

I checked the output.ps file and it definitely
is specifiying an A4 size output, even though
Kword has a custom page setup.

How can I print to a postscript output file
with a custom page size ?

Any help would be appreciated.
