I had this problem some time ago. I could only access my bookmarks via the "bookmarks" menu. The toolbar did show up in Konqueror, but it was empty. I solved the problem by adding a new bookmark. The new bookmark showed up, and so did the old bookmarks. The I deleted the new bookmark (as I only set it to see if it would work).
Perhaps you can solve your problem the same way.

But can you still see your bookmarks in the Bookmark Menu? If yes, try this:

Edit your bookmarks (Bookmarks menu > Edit Bookmarks)
The right-click on the topmost parent folder that includes all other bookmarks. In the default, the name of that folder would be Bookmarks. Click on "Show in Toolbar". If that doesn't work, you can try clicking on "Set as Toolbar folder".

Hope that works...
